CORI/SORI Process

Volunteer and Teacher Forms

All teachers working with children in Children’s Religious Education must complete a Background Authorization and Code of Ethics. 

Background Authorization
First Church Belmont requests Criminal Offender Record Information (CORI) and Sexual Offender Record Information (SORI) on all new hires and employees, as well as volunteers who will be working regularly with youth and children at First Church.

If you wish to volunteer in a position working regularly with youth and children, please provide the following information so that First Church can submit requests to the Department of Criminal Justice Information Services (DCJIS) and the Sex Offender Registry Board. Completion of this Authorization Form is completely voluntary, but your application to volunteer with youth and children at First Church will not be considered without it. (Even if you have done a CORI/SORI screen for schools, youth sports, etc., you must also do one for First Church). To complete the CORI/SORI screening, please print out and complete the following form: corisori.

To the return the completed form:

  1. Leave in the drop box outside the church administrator’s office
  2. Mail it to Janice Zazinski, FCB, 404 Concord Ave., Belmont, MA 02478
  3. Scan the completed form and send it to: info@uubelmont.org (please note that e-mail is not completely secure and you are sending personal information)
  4. Fax to the church (no cover sheet needed) at: 617-489-5341

Code of Ethics
Please read, print, and sign the Code of ethics. Return the signed form to Children’s Religious Education.

Volunteer Driver Forms
If you will be driving any children as part of a sponsored Children’s Religious Education activity, you must complete a volunteer driver form and return it to Children’s Religious Education. In addition, you must also complete a Background Authorization. (See instructions above for the Background Authorization).
